SPACE INFECTIONS

SPACE INFECTIONS

There are spaces present between the fascia (soft tissues) and underlying organs which actually do not exist but these spaces are created when there is a presence of infection or pathology. There are multiple spaces in the upper and lower portion of the face. Out of which the Submandibular Space infection is the life-threatening one.
Submandibular Space is at the lower border of the jaw. Infection from our posterior teeth drains into the submandibular region causing Sub Mandibular Space Infection, the more severe form is termed as Ludwig’s Angina. It is a rapidly spreading infection that leads to severe pain, swelling, dysphagia and can be fatal if left untreated.

SIGNS AND SYMPTOMS:

Initially, it starts with pain in tooth and indurations in the lower border of the jaw. And in the later stages, it progresses to swellings, tenderness, restricted mouth opening, dysphagia, sound on breathing and blockage of the airway due to the elevated posterior tongue.

TREATMENT PROTOCOLS

  • Maintenance of airway
  • Incision and Drainage
  • Antibiotic therapy

WHAT IS INCISION AND DRAINAGE

It is a procedure performed to treat the abscess. An abscess is the accumulation of pus. It appears as a painful, swollen area with a warm, red surface over the skin. It is treated by a procedure called Incision and Drainage also termed as I&D. It is puncturing the surface skin over the abscess and release the pus accumulated under the skin.

HOW DO YOU DO INCISION AND DRAINAGE..??

The surface is painted with a povidone-iodine solution and a stab incision is placed at the center of the abscess.  The pus and built-in pressure are released. Once the pus is completely removed, debridement is done with antibiotic solution. And the patient is advised to take oral medications for 3-5 days to get rid of the infection and prevent it from occurring again.
